All athletes must have their physical up to date prior to the first practice for their sport.
In order to practice or play a sport a student must have their updated physical paperwork completed and turned into the athletic office. We recommend that you schedule your student's physical in June or early July. By doing so, you will avoid the rush and your student will never have to worry about missing practice during the school year. Physicals are good for one calendar year from the date they are completed. These physicals should be completed by your family physician or pediatrician. Students without a current physical will not be allowed to practice or play.

Page 1-the Athletic Participation checklist
Page 2-the Agreement for Athletic Participation to be signed by the athlete and the parent
Page 3-the CIF Code of Ethics to be signed by the athlete and the parent
Page 4-the Athletic Consent/Insurance form to be completed and signed by the parent
Page 5-the History Form to be completed and signed by the athlete and parent
Page 6 and 7-the Physical Exam Form and the Clearance Form to be completed and signed by the student's physician.
Page 8-CIF Concussion RTP to retain for future use
Page 9 and 10-CIF Concussion Information Sheet to be read by the athlete and the parent and kept for reference
Page 11-CIF Acknowledgement of Concussion Information to be signed by the athlete and the parent
